Prove page template vs Forms block vs app embed vs CSS hide.
On Current theme (not a draft), open the Contact page URL in incognito. Confirm the shell loads but Name / Email / Message are missing or empty. Soft-check page / contact JSON template for emptied liquid or removed form markup. Soft-check Customize → page / contact templates for a disabled contact form section, Shopify Forms block, or app block. Soft-check App embeds that inject contact UI. Soft-check CSS / overlays that hide inputs. Soft-check Markets / password / customer-accounts gates that blank the form for guests.

Repair template / Forms block / embed; retest contact form.
Restore page / contact template from the theme default if liquid was emptied. Re-enable the contact form section, Shopify Forms block, or app block on Current and Publish. Disable conflicting form apps one at a time. Remove CSS that sets opacity:0 / display:none on inputs. Confirm guest vs logged-in access if customer accounts gate the form. Publish Current, hard-refresh on desktop and phone.
Honest retest: Name / Email / Message and Send are visible. Soft-link shopify-forms-not-submitting when the form shows but submit fails. Soft-link shopify-newsletter-form-not-submitting when newsletter signup is the leftover gap. Soft-link shopify-inbox-chat-not-showing when the chat bubble is the leftover gap.

The only honest test for this page: Contact / contact-form URL loads a blank or empty form shell — no usable Name / Email / Message — while related Forms-submit / newsletter / Inbox flows may still work.
