Shopify Forms / contact form not submitting — not Inbox chat, Klaviyo flows, Gorgias Rules.

“Form,” “lead,” and “submit” show up in three different problems. This page is only when the Shopify Forms app block or theme contact / lead form does not submit, does not create a customer / contact, or does not sync to notifications / Klaviyo. Shopify Inbox chat bubble missing on the storefront is shopify-inbox-chat-not-showing. A Klaviyo flow that should send but never fires is klaviyo-flow-not-sending. Gorgias Rules that should auto-tag or auto-reply tickets but do not fire is gorgias-rules-not-firing. App embeds that fail to load generally are app-embed-not-loading.

Prove Forms install vs app embed vs spam protection vs email vs Klaviyo sync.

Open Admin and a clean storefront tab for the same published theme. Capture five proofs before you blame “form SEO”:

  1. Forms / contact surface: is Shopify Forms installed (or is this a native theme contact form / page)? Any reconnect / permissions banner on the Forms app?
  2. Placement: Online Store → Themes → Customize on the published theme — is the Forms app block / embed or theme contact section present on the page customers hit (not only a draft theme or preview)?
  3. Spam protection: CAPTCHA, honeypot, rate limit, or “block disposable email” settings silently rejecting your test submit? Check Forms / app spam logs if available.
  4. Notification email: does Admin or the Forms app show the submission, while the merchant notification never arrives (wrong recipient, spam folder, Shopify notification template disabled)?
  5. Customer create / Klaviyo: is “create customer” / list sync off, Klaviyo disconnected, or a double opt-in / suppressed profile making it look like the form “did nothing”?

If you only tested Inbox chat missing, a silent Klaviyo flow, or Gorgias Rules, you are on the wrong playbook.

App embeds / theme sections: Forms block on the published Online Store page.

Shopify Forms on Online Store 2.0 is usually an app block or app embed on a specific page template — not something that appears storewide by magic. If the block was removed after a theme switch, lives only on a draft theme, or the contact page points at an old template without the form, visitors will see a dead or empty form area (or no form at all). Open Customize on the Current theme → the contact / landing page → confirm the Forms block or theme contact section is present, Save, then hard refresh the live URL (and an incognito window).

Theme contact forms (Liquid {% form 'contact' %}) fail differently: wrong form tag, missing submit button name, JS intercepting submit without calling the native form, or a page that is not a Shopify page template. Prove with a bare theme form before blaming Klaviyo. Theme editor preview with the form visible is not proof the published page has it.

Spam protection / CAPTCHA blocking submits, and notification email not landing.

Aggressive spam protection can look identical to “form not submitting”: CAPTCHA never completes on mobile, honeypot fields filled by autofill, disposable-email blocks, or IP rate limits on repeated staff tests. Temporarily relax spam settings, complete CAPTCHA carefully, and check Forms / app submission logs for rejected attempts. Also watch browser console for JS errors from other app embeds that abort submit handlers.

Separately: if submissions appear in Admin / Forms but you never get email, fix notification recipients, spam/junk folders, and Shopify notification templates for contact / form mail — that is not the same bug as Klaviyo marketing flows. Confirm SPF/DKIM for Shopify mail is healthy, then resubmit once with a fresh address you control.

Customer create missing and Klaviyo / list sync not receiving the form.

Shopify Forms can create customers / contacts and push to Klaviyo (or a list integration). If “create customer” is off, marketing consent is required but the checkbox is missing, or the Klaviyo connection / list mapping is stale after a reconnect, the form can “succeed” on screen while nothing appears in Customers or Klaviyo. Re-check Forms settings → customer creation + consent, reopen the Klaviyo (or ESP) integration, map the correct list, then submit a unique email and wait for sync lag before declaring failure.

Double opt-in, suppressed profiles, and GDPR / Customer Privacy consent can also hide the lead: the submit worked, but the profile is pending confirmation or suppressed. That is still this playbook’s sync path — not a silent Klaviyo flow that never triggers after a healthy profile exists.
