A cash on delivery / COD order confirmation is not a card authorization-pending notice and not a cancel receipt. Payment pending / authorization failed is payment pending email not sending. Order cancelled is cancel confirmation — order cancelled email not sending. Order edited is edit confirmation — order edited email not sending. Draft order invoice is unpaid draft — draft order invoice not sending. Gift card created is gift card email not sending. WISMO is “where is my order” — WISMO email not sending. Partial fulfillment / remaining items shipped is partial fulfillment email not sending. Refund confirmation is money moving back — refund email not sending. Abandoned checkout is unpaid cart — abandoned checkout email not sending. General paid-card order confirmation silence (any gateway) is order confirmation email not sending. This page is the buyer-facing path when a COD checkout completes and Shopify should email that confirmation: native Order confirmation under Settings → Notifications, or a Shopify Flow watching Order created with payment gateway Cash on Delivery.
Six ordinary reasons COD confirmation never lands: the order was never Cash on Delivery, Order confirmation is off or Flow is Draft / card-only, Payment pending status on COD was chased as a different product, sender/spam hid a send that did fire, you tested a different email product, or you never built a path for COD Order created. Walk them in that order. You already pay for Shopify notifications. You do not need a second SaaS to read notification history.

Order was never Cash on Delivery — card Paid, draft, or abandoned cart instead.
Open Orders → the test order. Payment method / gateway must show Cash on Delivery (or your installed COD app / local provider name) — not Shopify Payments card, PayPal, Shop Pay, a draft invoice, or an abandoned checkout that never became an order. A Paid card order that “felt like COD because we collect cash later” is not a COD confirmation event. A draft you meant to invoice is draft invoice mail. A cart that never checked out is abandoned checkout.
Common traps: COD payment method disabled in Settings → Payments so checkout never offered it; a custom checkout app that labels the order COD in a note but never sets the gateway; staff creating a draft and marking it Paid in cash without a real COD Order created; and testing on a market / shipping profile where Cash on Delivery is not available. Wrong gateway, wrong silence.
Prove it on a test order you own: enable Cash on Delivery for the market, place a cheap COD checkout to an inbox you control, and read Timeline for Order confirmation / notification activity plus Settings → Notifications → Order confirmation recent activity. No COD Order created means fix payments / checkout first — not the template.

Order confirmation template off, or Flow Draft / only matches Paid card gateways.
Open Settings → Notifications. Confirm Order confirmation is enabled — that is the native path for most COD checkouts as well as card. Open Shopify admin → Flow. Find any “COD confirmation,” “cash on delivery order,” or “Order created + Cash on Delivery” workflow. Status must be Active, not Draft. Conditions that only match payment_status Paid with Shopify Payments / card gateways, or that exclude payment gateway Cash on Delivery, will silently skip every COD order — card confirmations may have mailed while your COD path never saw the order.
Marketing campaigns titled “Thanks for your COD order” in Shopify Email or Klaviyo are not the transactional path. Building a campaign does not fire when a specific COD Order created lands. Mixing campaign email with notification / Flow actions is how you pay for three products and still send none on COD. If Flow never runs on any trigger, that is the broader Shopify Flow not running playbook. This page assumes COD orders exist and asks why the confirmation path stays quiet.

Payment status Pending on COD confused with payment-pending authorization mail.
Many COD orders sit Payment pending until cash is collected on delivery. That status label is not the same product as card authorization failed / payment pending customer mail. COD still needs Order confirmation (or a COD-aware Flow) at Order created. Chasing the payment-pending template while Order confirmation is off is how you “fix pending forever” and still never email the COD buyer that the order exists.
Read Timeline for the order ID. A COD Order created with no “Order confirmation email sent” line is not “Shopify never emails COD” — it is confirmation off, Flow skipping COD, or wrong gateway. If you truly need authorization-failed / pending payment mail for card checkouts, that is payment pending email not sending— a different playbook.

Sender / spam / promotions hid a send that did fire.
Timeline can show Order confirmation email sent while the COD buyer swears silence. Check the From domain / SPF / DKIM for the store, Gmail Promotions vs Primary, and whether the subject looks like marketing (“pay cash on delivery — special offer”) so filters bury it. Force one test COD checkout to an inbox you control. Search spam for the exact subject your template uses.
If Timeline says sent and the inbox is empty after spam checks, fix deliverability — do not buy another notification app to paper over a domain that already fails DMARC on order confirmation.

You tested payment pending, order cancelled, order edited, draft invoice, gift card, WISMO, partial fulfillment, refund, or abandoned checkout.
Leaving a card Payment pending is payment pending. Cancelling the order is order cancelled. Editing the order is order edited. Sending a draft invoice is draft order invoice. Issuing a gift card is gift card. Asking “where is my order” is WISMO. Shipping remaining lines is partial fulfillment. Issuing a refund is refund. Leaving a cart unpaid is abandoned checkout.
The only honest test for this page: Cash on Delivery enabled, a real COD Order created to an inbox you own, Order confirmation / COD-aware Flow Active, then Timeline + inbox. Anything else is a different product.
